Services through Division of Senior Services (DSS) also provide support to enable people to live as independently as possible within the community setting of their choice in a non-institutional environment. These services are also provided for adults with physical disabilities. The services provide an opportunity for the family member to remain in the home as long as possible with assistance. These services include but are not limited to:

Key Benefits

Care Coordination - a person or team works with consumer and their families to develop an appropriate service plan to assure and monitor access to services and advocate for the individual and their family.
 
Daily Respite / Hourly Respite – allows the primary caretaker some relief while also giving the client time to socialize with other people.
 
 Chore - Someone will assist with household out door chores, which have become difficult of impossible due to disabilities.
 
Environmental Modification – physical adaptations to the home to enable recipients to function with greater independence.
 
Transportationto gain access to community services & resources.
